General appearance
: Lethargic
Vital signs

Pulse: 200 beats/min
Respirations: 68 breaths/min
Weight: 5.7 kg (12.5 lbs)
Skin
: Wound on left forelimb, hair coat normal but not well groomed
HEENT
: Mucous membranes are very pale. Eyes are slightly sunken into orbits. Ears have mild debris and are cool to the touch. No submandibular lymphadenopathy or oral lesions observed.
Pulmonary
: Eupneic with no abnormal lung sounds
Cardiovascular
: Capillary refill time >2 seconds; grade 2.6 left parasternal systolic murmur
Gastrointestinal
: Gut sounds present but decreased. Abdomen soft and not painful.
Genitourinary
: Urination not observed but no apparent abnormalities
Musculoskeletal
: Grade 1/5 lameness of left forelimb
Neurologic
: No abnormalities noted
Mental status
: Lethargic and uninterested in surroundings
