Social history
: Patient is a student at the local community college. He denies current and previous tobacco use; he drinks a 6-pack of beer on weekends and denies illicit drug use. He exercises several times a week. All relevant immunizations are up-to-date. He denies any recent travel. He denies exposure to any sick contacts. He is sexually active with four partners and does not use condoms.
